Returning Students - Please fill out the online registration and provide us with the Online Registration number via this google form. Nothing else is needed at this time unless your student has a temporary medical or religious immunization exemption on file, a new one will be needed annually.
New to CCSD, Kindergarten or new to Brown ES and current students needing to fill out a new waiver-Register online and write down your application number. Beginning on May 26th you may bring your Application number and required documents to our office from 9:00 AM-1:00 PM.
New Waiver Changes
The religious exemption form will be completed annually in accordance with state guidelines.
The permanent medical exemption will not have to be completed annually since it is a permanent medical condition.
The temporary medical exemption will be completed annually in accordance with state guidelines.
